The Arts & Culture leadership team is committed to providing high quality, inspiring leadership. As part of this commitment we are seeking your feedback about how we are currently performing in the leadership space.  

This 360-degree survey describes the critical leadership behaviours we want to see our leaders excelling at.  You have been asked to complete this survey because we believe you will bring valuable insight to this person around what they are doing well and their opportunities for improvement.

Please read each of the statements carefully and make an assessment of the person's performance by providing honest, constructive responses. There are no right or wrong answers, you have been chosen as a feedback respondent because this person is interested in what you have to say. You will have the opportunity to rate the person's performance using a 6-point scale - please try to avoid using only one part of the scale i.e. all 5s.
