As part of our team's commitment to being the best leadership team we can be, we are interested in understanding how we are currently performing in the leadership space. On the following pages there are statements describing critical leadership behaviours. Please read each of the statements carefully and make an assessment of the person's performance by providing honest, constructive responses on the form provided. There are no right or wrong answers, you have been chosen as a feedback respondent because this person in interested in what you have to say. You will have the opportunity to rate the person's performance using a 6-point scale. Please try not to use only one part of the scale - e.g. all 5s.  Instead try to use the scale to differentiate the things the person does well and where they can improve. You will also be asked for written feedback - please be as specific and descriptive as possible in these sections, people always find this feedback most revealing and helpful.

The survey will take you time - you will need to allow approximately 20-30 minutes to complete a survey. We thank you in advance for investing in the development of this person.
